Health checkups for daytime students who have transferred to other academic programs, including early admissions to graduate schools, in the second semester of the 111th academic year

I. Basis:

Ministry of Education "School Health Law", "Implementation Rules for Student Health Examination" and our "Health Examination and Disease Prevention Points" and other relevant regulations.

II.Target: Students who have not undergone physical examination in each academic structure

*Students who have been suspended from school for more than 2 semesters are required to take a new physical examination.

*If you have already taken a physical examination, please note that we only accept medical reports within 3 months.

(If the date of physical examination is before November 1, 111, the report will not be accepted, please re-examine.)

III. Examination date: February 22, 112 (Wed) 13:30~16:30 (16:10 deadline for registration)

IV. Place of examination: 1st floor of the Student Activity Center, inside the student cafeteria.

V. Health Screening Fees and Payment Methods:

(1) The fee for the medical examination is 470 yuan per person. Please pay to the hospital on the day of medical examination and keep the payment certificate by yourself.

(2) The examination is free for those who have a certificate of coldness issued by the district office and low-income households.

VI. Health Examination Items:

(1) Please refer to the student health information card (English Student Health Information Card)

(2) All items of student health examination are based on the regulations of the Ministry of Education, therefore, the specifications of military service physical examination or labor physical examination cannot be used.

(3) Please paste a 1-inch photo on the student health information card and fill out the front of the student health information card for the examination, and the back of the health examination record form will be filled out by the hospital.

VII. Notes on Health Screening:

(1) Please bring your ID card, $470 medical examination fee, student health information card, and consent form to be examined.
  *The medical examination information for reinstated students has been sent along with the "Notice of Reinstatement".
  *The medical examination information for transfer students has been sent together with the "Registration Information".

(2) Please maintain a normal work schedule and do not overeat 3 days prior to the health examination to avoid affecting the examination results; no fasting is required before the examination.

(3) If you have regular medications for chronic diseases, please take them normally.

(4) If you are pregnant, it is recommended that you do not have X-ray examination.

(5) Please refrain from wearing contact lenses and avoid wearing clothes with metal objects (buttons, necklaces, sequins).

(6) If you are unable to attend the physical examination on campus, please bring your physical examination card to the qualified hospital of the Ministry of Health and Welfare and submit the physical examination report to the Health and Safety Section of the Office of Student Affairs by February 24, 112 (Fri).

(7) The student health examination is implemented in accordance with the "School Health Law" and "Implementation Regulations for Student Health Examination" of the Ministry of Education. If the student fails to complete the health examination and causes a public safety problem on campus, it may be related to Article 62 of the Infectious Disease Prevention and Control Act, and the most serious penalty is imprisonment for up to three years, detention, or a fine of up to NT$500,000.

VIII. Remarks

1. Two copies of the health examination report will be kept by the Health Center and the student, and the student part will be issued to the student by the instructor. If there is any abnormality in the examination result, please review and report to the health center.

2. The Health and Safety Section will establish a student health database based on the student health examination results, and will conduct health tracking management and health promotion activities for reference.
