Hello Faculty and Staff
We welcome all faculty and staff members who are eligible for the "111th Annual Influenza Vaccination" program to apply.
I. Target population
(1) Faculty and staff members who are 50 years of age or older (born 61 years ago), and whose age is calculated as "year of vaccination" minus "year of birth".
(2) Pregnant women. (Proof of mother's handbook is required)
(3) Parents of infants under 6 months of age. (The baby's handbook is required)
(d) Kindergarten caregivers and professional staff of child care institutions.
(5) Medical and health prevention personnel. (Supporting documents are required for practice license)
Date: October 19, 111 (Wed), 15:30-16:00 pm.
Location: Multi-purpose Exhibition Center.
4. Seeding Institution: Tainan Branch of Kaohsiung Veterans General Hospital.
V. Registration: From now until 12:00 noon on October 12, 111, please complete the registration at the school's "Online Activity Registration Information System" (http://action.tut.edu.tw/) to facilitate the subsequent registration process.
6. Please bring your "Health Insurance Card" and "Staff Card" on the day of enrollment.
