2. Place for the physical examination: the square in front of the GUNYI Hall and the student meal (1F of the Student Life Activity Center)

3.Please have your ID card, the medical examination fee of $470, student health information card and consent form (please fill in the information before you report for the medical examination) to facilitate the examination.

4.If the student health information card and the consent form are lost or not brought, please pick them up at the Health and Safety Section before 17:00.

5. Those who have a certificate of coldness issued by the district office or a low-income household can be examined free of charge, please submit the certificate to the physical examination registration office.

6. No fasting is required before the examination. If you are prone to dizziness, please inform the medical staff before drawing blood; the physical examination can be started from the station with fewer people to avoid long waiting time.

7. Student health information card and consent form must be submitted to the physical examination card return office before the physical examination is completed.
